Team Captain Registration Form
January 3 –
March 6, 2008
Team Captain Responsibilities / Requirements:

Gather a team
of 4-10 people before December 6, 2007
(A team can be made up of co-workers, neighbors or adult
family members.)

Collect and
Return Team Member Registrations and Entry Fees
(Entry fee of $20 per person; scholarships are
available).
-
Appoint a
co-captain to assist the captain with
responsibilities.

Attend a
Captain’s Meeting on December 6, 2007 OR December
13, 2007 at 6:00 pm
Maine Coast Memorial Hospital Classroom (Park in the
Acadia or Cadillac parking lots, watch for signs)
OR in Deer Isle Wednesday, December 5, 2007 at Sunset
Parish House

Select a team
name that is fun and inspiring by December 6, 2007.

Set up a team
goal i.e. weight loss, increase physical activity,
quit smoking, etc.
-
Remind team
members to track progress for a healthy lifestyle
change
-
Attend as many
weekly meetings with your team as possible in
Ellsworth (Thursdays at 6:OO pm) OR Deer Isle (
Wednesdays at 6:00 pm)

Be able to
contact your team members easily to:
- Collect team member weights -Arrange carpooling - Be a
cheerleader
- Distribute weekly recipes and healthy hints - Provide
support

Report team
weight each Monday, no later than 2 pm (a must)

Complete this
form: No later than
November 30, 2007
Once you have
completed this form, we will make an appointment with you to pick up your
Captain's Kit and pay registration fees. |
