The following services are available to
help you with End of Life and Palliative Care.

MCMH
Services
Palliative Care Team including nursing, hospice and
pharmacy specialists review patient's care.
Susan Ostertag, MD works in collaboration with
primary care physician to provide maximal comfort, autonomy
and symptom relief to those patients nearing the end of life.
Work is done in conjunction with community hospice resources
including Hospice of Hancock County (Volunteer) and Community
Health and Counseling Services (Medicare benefit).
