The geriatric Assessment Clinic at Maine
Coast Memorial Hospital serves as both a diagnostic
services for the diseases and ailments of the elder
patient and as a coordinating and referral services for
those dealing with geriatric conditions.
About Our Office

Location...The Geriatric Assessment Clinic
is located in Suite 2200 in the Medical Office Building at Maine
Coast Memorial Hospital.

Appointments
are available by referral from your physician Monday through
Friday from 8 AM to 3 PM.

Should
you need special help entering our offices, please drive to the
hospital's main entrance where the Parking Volunteers will
assist in parking your car and transporting you to the office.
Services
- Evaluation of memory disorders and
dementia Memory disorders and dementia evaluations
are performed. Full mental and functional assessment done
jointly by physician and nurse with geriatric training.
Includes diagnosis, recommendations for management and
referral to appropriate community resources in collaboration
with primary physician. Competency evaluation included when
appropriate.

Our Discharge Planning Team
follows patients home with a discharge plan to ensure
