Ellsworth
Internal Medicine would be pleased to be your personal

important decision. The information on this web page is
provided to assist you with your selection process and
to answer questions commonly asked. However, if you have
other questions please call the office or ask any staff
member to address your inquiries.
Ellsworth Internal Medicine was founded in 1981. Over
the years, thousands of men and women have benefited
from the outstanding health services offered here.
All practitioners with Ellsworth Internal Medicine are
board certified internist, certified physicians
assistants and nurses. We enjoy caring for the full
range of adult medical needs and preventive care.
About Our Office

Emergencies and Urgent Care Any
medical emergency should be transported
immediately by ambulance if possible to
the nearest emergency room. The
Emergency Room at MCMH is open 24 hours
a day and has a direct phone line
664-5340. Medical problems you believe
need urgent attention will be seen in
our offices if possible that same day.
If for any reason your individual
practitioner is unable to see you, we
will make every effort for you to be
seen by another practitioner, or the
Emergency Room (664-5340).

When you
Come for Your Appointment
Bring all of your medications in
their original containers, including eye
medication, nonprescription medications
and vitamins you are currently taking.
Wear comfortable clothing.
Please do arrive early.
Remember Insurance Information.
Bring your insurance cards with you.

Referrals
to Specialists We believe that
everyone should have one primary
physician who takes care of most of
their health care needs. However, on
occasion we need to refer patients to
our colleagues in areas of specialized
medicine. For this reason we maintain
close working relationships with
specialists at Maine Coast Memorial
Hospital and elsewhere.

Phone
Calls When calling our offices
we will transfer your call to the staff
person best able to provide the answer
to your questions or inquiries. Health
care insurance covers some, but not all,
of your treatment. Most plans include
co-insurance, a deductible and other
expenses, which must be paid by you. If
you have insurance, we will bill your
insurance from our office. Please bring
your insurance card with you to assure
you receive maximum benefits to which
you are entitled.

Test
Results We will contact you with
your test results either by telephone or
mail as soon as they return to our
offices. If for any reason you have not
heard from us, please feel free to all
so we can check on them for you.

Prescriptions We are happy to
refill prescriptions during your
appointment. Should you need refills
between appointments, please call your
pharmacy. They will in turn call our
office to confirm the medication and
obtain the authorization for the refill.
Please make every effort to allow a 24
hour advance notice to the pharmacist.

Hospitalization We are part of a
shared practice. If you are hospitalized
one of the physicians in our group will
be responsible for seeing you every day.
The physicians met daily to discuss
hospitalized patients. Once released
from the hospital you will continue to
see your regular physician.

Privacy
and Confidentiality Your medical
record is strictly private. Unless
required by law, no information
regarding your condition will be given
to your employers, friends, or
relatives, without your permission. For
your protection we will release such
information only with your written
approval.

Fees and
Payments We’re making every
effort to keep fees reasonable. Because
of billing, statements and procession of
insurance claims are expensive and time
consuming, you can help us contain by
paying any co-payment or deductible when
services are rendered.

Insurance
Managed Care: If you are covered
by managed care insurance and we are
your PCP (Primary Care Physician),
remember to call our office for any
referrals to other physicians, physical
therapy, x-rays, etc. If we are not
notified in advance, your insurance may
not cover the referral.
Health
care insurance covers some, but not all
of your treatment. Most plans include
coinsurance, a deductible and other
expenses which must be paid by you.
If you have insurance, we will bill
your insurance from our office. Please
bring your insurance card with you to
assure your receive the maximum benefits
to which you are entitled.
If you
have difficulty paying your bill, or if
you have questions about your bill,
please call our billing office at
667-7065.
Please notify our
patient representatives at the time of
your visit if you have changed your
insurance plan or your address and phone
number, so we can update our records.

Health Education—Our nursing staff is
specially trained to work with you in
your understanding of a good and healthy
life style, an important part of today’s
healthcare.
Our patients range in age from
adolescents (starting at age 15) to
elderly. Our medical practitioners also
care for our patients in the hospital,
the emergency room and at local nursing
homes.
Our board certified internists are
physicians who have received additional
training in adult medicine with emphasis
on the various sub specialties. Dr.
LaRocco is board certified in
Geriatrics. Our certified physician
assistants have undergone several years
of training in general adult medicine
and work under the supervision of our
physicians.
