The staff and physicians at the Eleanor Widener Dixon Memorial Clinic, Ellsworth Internal MEdicine and Ellsworth Family Practice are successfully of partnering with at-risk patients. The Collaboration is an important model for quality management. It focuses on individuals with diabetes, obesity and/or cardiovascular disease. Through a central registry, both physicians and patients receive reminders and report cards that monitor blood pressure, body mass index, blood sugar levels, good and bad cholesterol, triglycerides kidney/liver problems and give foot and eye exams. 

The Eleanor Widener Dixon Memorial Clinic and Ellsworth Family Practice are two of 15 selected sites statewide that will take part in a system which is proving to have early success in lowering the incidents of cardiovascular events, strokes and loss of limbs by as much as 10%. This is done by aggressively managing lifestyles and risk factors. Physicians and staff alike are excited about the relationships and promises for better outcomes for their patients.
