A Proven Model for Improving Health Care Outcomes

The staff and physicians at Maine Coast Memorial Hospital are successfully partnering with at-risk patients. The Collaboration is an important model for quality management. It focuses on individuals with diabetes, obesity and/or cardiovascular disease. Through a central registry, both physicians and patients receive reminders and report cards that monitor blood pressure, body mass index, blood sugar levels, good and bad cholesterol, triglycerides kidney/liver problems and give foot and eye exams.

All Maine Coast Physician Practices take part in a statewide system which is proving to have early success in lowering the incidents of cardiovascular events, strokes and loss of limbs by as much as 10%. This is done by aggressively managing lifestyles and risk factors. Physicians and staff alike are excited about the relationships and promises for better outcomes for their patients.

Diabetes education and nutrition therapy are important parts of this program. RSS
